While the simple answer is that it takes this long to complete a throat culture. many other aspects of this illness are raised by this student's straightforward question. Not only the relief from a painful condition. but the protection of an indiyidual from complications. and the identification and control of a potentially epidemic disease are involved. Historically. the streptococcus sore throat has been a dangerous disease. Complications. some of them fatal. include meningitis. septicomia (generalized infection of the blood stream). mastoid infection and scarlet fever. Long-term serious effects are nephritis and rheumatic fever which can be crippling for the patient. With the development of sulfa drugs and penicillin it has now become possible to cure practically all cases of strep throat and prevent its complications. The streptococcus is a widespread bacteria commonly found in the human respiratory passages. It is spread by droplet spray during talking or coughing. It can be transmitted by inert materials like milk- one reason for pasteurization. Symptoms of infection include fever. headache. sore throat. earache. rash. and swollen glands. It should be realized that many of these mannifcstations are found with "viruses" and even mononoclcosis. It is. of course. important to identify people with strep throat because there is a specific drug (penicillin) which will give the patient fairly quick relief and prevent the dangerous complications. Most of the time. however. students seen in the health service do not have all those complaints (and other physical findings) so that it is unlikely that they have the real thing. Under those circumstances. it is safe to take a culture and wait the two days incubation to be sure before prescribing penicillin. An important aspect of taking throat cultures is to monitor the incidence of streptococcal disease in our student population. There are always a few cases around. But we must be on the alert against a significant increase in its incidence due to rapid spread and the possibility of an epidemic with many persons laid up at the same time. So. when you come to the Student Health Service for a sore throat. be assured that your case will be carefully evaluated. and suitable medication prescribed according to the findings. antibiotics may be used immediately or perhaps only after a confirmatory culture with due consideration for your comfort and welfare as well as the health of your fellow students. He may also be considered the new Paul Newman. and I say that with no hesitation. He even looks a bit like Newman - note the shape of the eyes. However. although Rydell has access to good actors and act- resses. he lacks a major sensibi- lity necessary in the film medium - literary sense. It is evident in all his work that his main concern is the acting - which is no surprise. as he was. and still is, an actor himself.‘ But Rydell is no Ernst Lubitsch. ‘ or even a Mel Brooks (for lack of a better contemporary example). when it comes to telling and pacing a story. “Harry and Walter” takes much too long to build up to the main plot. and the premise it is built on is one of dubious credibility to begin with. Much unnecessary detail could have been cut out towards the beginning: for instance. the busi- ness of selling locks of Adam Worth‘s hair. The final scenes are the best. however. with a very well-timed integration of Vaudeville and safe-cracking. As it may be observed with Mark Rydell. his productions have a big-budget look to them, wide-screen. and careful set de- sign. As a fact. Rydell is extreme- ly meticulous with detail. and “Harry and Walter"‘ gave him his first chance with a difficult period. Rydell has shown a talent in creating the appropriate atmosphere. not unlike Robert Altman. Most of the problems with “Harry and Walter" lie in the script. One wonders how come Diane Keaton. as the WASPish editor of the small newspaper. ever came to the head of a staff of Bohemians. Russians. and Slavs. They all have heavy accents except for her. and this element is emphasized in the casting of Carol Kane (“Hester Street") and Jack Gilford. Certainly Kane was cast be- cause of her much publicized “turn-of-the-century" looks. But Jack Gilford? - a highly accom- plished film comedian (“A Funny Thing Happened On _The Way To The Forum") as well as a fine dramatic actor (“Save The Tiger"). His character is de- emphasized so much it seems very likely his part was chiseled down from a relatively important role to one less visible. and most likely to justify the billing of Diane Keaton. who] have always said is not that good an actress. A problem such as this is one of commericalism -Rydell's second big weakness. He doesn't even have the occasional guts of Paul Mazursky. director of “Harry and Tonto" - a film of which I am sure Rydell is very jealous. He can continue making films with popular stars. but as l have implied. only one in the film. James Caan. contributes. while the others. with the possible exception of Michael Caine, de- tract from the quality of the film. Rydell's evolution in the ci- nema is unique in that he is a genuine talent. But it is also most common. in that he began as an idealist. and has steadily strayed from a successful working ideal- ism. for some unknown reason. over the years.
